Insurance and Privacy

IRC is in network with the following insurance companies:

Aetna
Blue Cross Blue Shield PPO
Blue Choice

If you do not see your insurance company listed above you will be expected to pay the full fee for services and will need to check with your insurance company for out of network benefits.

Preparing for your first appointment when using insurance.

  1. On the back of your insurance card is the number in which to call to speak with a customer service representative to verify benefits. Please ask for in network benefits and out of network benefits and confirm whether Michele Martin, LMFT is in network. Her name is used as she is the owner of IRC.
  2. Questions that should be asked are:
    1. What is the effective date of coverage?
    2. What is the annual deductible and how much has been met?
    3. What is the percentage paid after the deductible has been met?
    4. What is the co-payment or co-insurance amount?
    5. Is a referral from a Primary Care Physician required?
    6. Where do I mail Behavioral Health Claims?
    7. Is precertification required?

Authorization/Precertification is required.

    1. What is the authorization number?
    2. What is the authorization start date?
    3. How many sessions are authorized?
    4. Where do we mail claims? (This may be different than the address asked for above.)
